CARE MANAGER
Job Summary
The Resident Care Manager is responsible for the day-to-day unit operation including direct supervision of unit staff. This role interacts with physicians, nursing staff, other Home unit staff, and other departments and interdisciplinary team members on a daily basis and provides clinical support to patient/resident care by exception only. The position facilitates and fosters interdisciplinary participation in patient/resident care activities and coaches, mentors, motivates and empowers clinical staff. The Unit Manager creates a climate of interdepartmental respect and collaboration, and adheres and models the Covenant Health mission, vision and values. The responsibilities of this position are focused around four critical aspects of care and service: People and teams. Access and flow. Quality and safety. Efficiency and resources. This position is non-unionized.
Duties and responsibilities
As a Home Care Manager you will have many duties and responsibilities to ensure you provide the best care to our patients, including:
- Supervice Travels to patients’ homes to provide medical and personal care
- Manage Administering of medicine across the Unit
- Provide overal care e.g Inspect wounds and change dressings
- Oversee adherence to standards e.g Assist with personal grooming and hygiene tasks
- Manage Tests done at Unit Level e.g for things like infection, weakness, and bedsores
- Collaborate with Physicians to create personalized healthcare plans
- Help educate other Nurses,caregivers and family members on how to care for the patient
- Monitor patients and keep their treating physician in the loop
Education & experience requirements
- Age 25-55 Years Both Male & Female
- High school Certificate
- Nursing/ Caregiver Certified by a CNA Accredited
- Minimum 24 Months Experience + On Job Training
- In addition to the necessary education, Candidates with experience in a traditional nursing role or other large care setting with hands-on experience shall have added advantage.
- International candidates to attach Passport, PHI & Medical Board certifications
